`
`Plaintiff, Dewayne Cornett, is hereby ordered to pay as child support $391.00 per
`month for the minor child, Chase Cornett, through the Arkansas Child Support Clearinghouse.
`In addition, Plaintiff is ordered to pay $81.25 per month for medical insurance coverage to
`Defendant. Plaintiff is to pay the Arkansas Child Support Clearinghouse’s administrative cost
`of $36.00 per year. Plaintiff's employer is hereby ORDERED to deduct from Plaintiff's wages
`$391.00 per month for child support and $81.25 per month for medical insurance coverage.
`On each check for the child support, the employer shall indicate that the check is for child
`support and medical insurance coverage for case number DR-2008-404-2 and shall mail the
`check to the Arkansas Child Support Clearinghouse at P. O. Box 8125, Little Rock, Arkansas,
`72.203, and forward to Amy Cornett at 20309 U.S. Highway 71, Fouke, Arkansas 71837. In
`addition, Plaintiff's employer shall deduct $36.00 per year from Plaintiff's wages and remit
`said amount to the Arkansas Child Support Clearinghouse for its administrative fees.

`weekend of each period of extended summer visitation or in the alternative the
`second and fourth weekends that the child is in the possession of the Non-
`Custodial parent, beginning at 6:00 p.m. on Friday and ending at 6:00 p.m. on
`Sunday. The parents will meet at a public location that is approximately one-half
`(4) the distance from the city of the residence of the Non-custodial parent to the
`city of residence of the Custodial parent for the exchange of the child for extended
`summer visitation and for weekend visitation.
`
`Without Written Notice: Non-custodial parent shall have possession of the child
`beginning on at 6:00 p.m. on May 31” of each year or the last day of the regularly
`scheduled class at the child’s school and continuing for a period of twenty-one
`consecutive days with this period of visitation ending at 6:00 p.m. on the 21” day.
`The second period of visitation will begin at 6:00 p.m. on July 1" of each year and
`continue until 6:00 p.m. on July 21“ of each year. The Custodial parent shall have
`weekend visitation on the second weekend of each period of extended summer
`visitation that child is in the possession of the Non-Custodial parent, beginning at
`6:00 p.m. on Friday and ending at 6:00 p.m. on Sunday. The parents will meet at a
`public location that is approximately one-half (2) the distance from the city of the
`residence of the Non-custodial parent to the city of residence of the Custodial
`parent for the exchange of the child for extended summer visitation and for
`
`weekend visitation.

`child's birthday, Non-custodial parent shall have possession of the child beginning

`8. Mother's Day. On Mother's Day each year, the Mother shall have visitation
`beginning at 9:00 a.m. and ending at 6:00 p.m., provided that if she is not in
`possession of the child, she shall pick up the child from other parent's residence
`and return the child to that same place unless that is the last day of a period of
`
`possession by the father.
`
`9. Father's Day: On Father's day each year, the Father shall have visitation beginning
`at 9:00 a.m. and ending at 6:00 p.m., provided if he is not in possession of the
`child, he shall pick up the child from the residence of the other parent, and he will
`return the child to that same place unless that is the last day of a period of
`
`possession by the mother.
`
`Notwithstanding the weekend periods of possession ORDERED AND DECREED for
`Non-custodial parent, it is explicitly ORDERED AND DECREED that Custodial parent shall
`
`have a superior right of possession to the child as follows:

`Christmas Holidays in Odd-Numbered Years. In odd-numbered years, beginning at
`
`6:00 p.m. on the last school day before the Christmas school vacation begins and
`ending at 3:00 p.m. on December 25.
`
`Christmas Holidays in Even-Numbered Years. In even-numbered years, beginning
`at 3:00 p.m. on December 25 and ending at 6:00 pm. on the day before school -
`
`resumes.
`
`Thanksgiving in Even-Numbered Years, In even-numbered years, beginning at 6:00
`p.m. on the Wednesday before Thanksgiving and ending at 6:00 pm. on the
`
`following Sunday.
`
`Spring Break in Odd-Numbered Years. In odd-numbered years, beginning at 6:00
`p.m. on the last school day before the school's spring vacation begins and ending at
`
`6:00 p.m. on the day before school resumes.
`
`Child's Birthday. If Custodial parent is not in possession of the child on the child's
`birthday, Custodial parent shall have possession of the child beginning at 6:00 p.m.
`and ending at 8:00 p.m. on that day, provided that Custodial parent picks up the
`child from Non-custodial parent's residence and returns the child to that same
`
`place.
`
`Mother's Day. On Mother's Day each year, the Mother shall have visitation
`beginning at 9:00 a.m. and ending at 6:00 p.m., provided that if she is not in
`possession of the child, she shall pick up the child from other parent's residence
`and return the child to that same place unless that is the last day of a period of
`possession by the father.
`
`Father's Day: On Father’s day each year, the Father shall have visitation beginning
`at 9:00 a.m. and ending at 6:00 p.m., provided if he is not in possession of the
`child, he shall pick up the child from the residence of the other parent, and he will
`return the child to that same place unless that is the last day of a period of
`
`possession by the mother.

`(d) Parents Who Reside Over 100 Miles Apart
`Except as otherwise explicitly provided in this Standard Visitation Order, if Non-custodial
`
`parent resides more than 100 miles from the residence of the child, Non-custodial parent shall
`
`have the right to possession of the child as follows:

`Weekends. On every other weekend, beginning at 6:00 p.m. on Friday and ending
`at 6:00 p.m. on the following Sunday. Alternatively, the Non-custodial parent may
`elect to have visitation only one weekend per month on a weekend of that parent's
`choosing. However, the Non-custodial parent must make that election within
`ninety days (90) of this order being entered or from the change of his residence. |
`The parties shall meet at a public location one-half (4) the distance between the
`city of residence of the Non-custodial parent and the city of residence of the
`
`Custodial parent.

`Christmas Holidays in Even-Numbered Years. In even-numbered years, beginning

`noon on December 26" and ending at 6:00 p.m. on the day before school
`resumes. The parties shall meet at a public location one-half (%) the distance
`between the city of residence of the Non-custodial parent and the city of residence
`of the Custodial parent.

`Thanksgiving in Odd-Numbered Years. In odd-numbered years, beginning at 6:00
`p.m. on the Wednesday before Thanksgiving and ending at 6:00 p.m. on the
`
`following Sunday. The parties shall meet at a public location one-half (%) the
`distance between the city of residence of the Non-custodial parent and the city of
`
`residence of the Custodial parent.
`
`Spring Break in All Years. Every year, beginning at 6:00 p.m. on the day school
`
`recesses and ending at 6:00 p.m. on the day before school resumes. The parties
`shall meet at a public location one-half (%) the distance between the city of
`residence of the Non-custodial parent and the city of residence of the Custodial
`
`parent.
`
`Extended Summer Possession by Non-custodial parent.
`
`Written Notice: Non-custodial parent shall make an election on or before May 1*
`
`each year to the date or dates of visitation. The Non-custodial parent is awarded
`
`visitation of forty-two days (42) not to be exercised in more than two periods of
`three (3) weeks. The Custodial parent shall have weekend visitation on the second
`

`alternative forty-two (42) days for extended summer visitation, the Custodial parent
`may have visitation with the child on the second weekend of each period or the
`second and fourth weekends of extended visitation beginning at 6:00 p.m. on
`Friday and ending at 6:00 p.m. on Sunday. The parties shall meet at a public
`location one-half (44) the distance between the city of residence of the Non-
`custodial parent and the city of residence of the Custodial parent.
`
`Child's Birthday. If Custodial parent is not in possession of the child on the child's
`birthday, Custodial parent shall have possession of the child beginning at 6:00 p.m.
`and ending at 8:00 p.m. on that day, provided that Custodial parent picks up the
