`APPEARANCE OF A CORPORATION, PARTNERSHIP,
`ASSOCIATION, TRUST, LIMITED LIABILITY COMPANY, OR OTHER ORGANIZATION
`If the order to appear for the examination on page 1 does not require the appearance of a specified
