`directly to the court officer at (860)548-2814. Said pro hac vice attorney does not have efiling privileges
`but must have local counsel efile any subsequent documents.

`2. The attorney appearing pro hac vice is to notify the Commissioner of Revenue Services of this
`appointment.

`3. The attorney appearing pro hac vice is to pay all applicable attorney occupational taxes.
