`
`JOBSITE DESCRIPTION
`
` I had the chance to visit where the incident occurred. At the time of the visit the area was already paved. They were milling in
`Together with
`hill area slightly downhill in term of grade, side to side, depth was around 30 mm. The width of the street was 7.5 meters and they did 4 passes.
`When the machine tilt over they were doing the last pass close to the guard rail on the right. In this area the cross slope was pretty relevant, below
`the measurements taken on the job site. We detected the exact point where the machine tilt over, then we took the cross slope value every 2
`meters. On average the cross slope of the street in this area was 15.4% matching with 8.55 degree. In the previous 3 passes the machine worked
`with 13,5% of cross slope.

`MACHINE CONDITION

`Machine has been inspected in the yard by
`switched on after removing the engine oil from 2 inner liners. Machine has been tested without reporting any malfunctions, testing the legs
`functions and G&S system. No errors reported by the machine, no hydraulically or mechanical issues have been found.
`Checks all the G&S parameters, the parameters were as per standard setting, no weird configuration has been found.
`
`CONCLUSION
`
`It seems pretty clear the cross slope of the job site can lead to think in a operator mistake or inattention. The operator report a random machine
`movement probably because few days before entering into the cut they experienced in the front leg lift due to the issue related to antislab or
`moldboard that we are already working on CPI. The customer complains strongly about missing of anti rollover system that can prevent those
`incidents, he is reporting Wirtgen have it (need to be verified). He is refusing to use the machine until we have this system installed.
