`Introduction
`
`The purpase of this Construction Cuality Control Inspection Program is to provide a reference guide
`for maintaining consistent and effective quality control procedures during construction of the Yelm
`Hydroelectric Project Fish Screen and Spillway Mo. 3. As the owner, the City of Centralia will be
`solely responsible for developing and executing this program,.
`
`This plan has 5 major sections;
`# Section | explains the major project elements.
`
`* Sechion I describes the quality control inspection and testing program which includes field wests,
`field laboratory facilities and the planned use of commercial testing services.
`
`* Section } details the construction management organization, including the roles and
`responsibilities of the project team members
`
`# Sechon 4 describes the schedule of construction showing all major features of the project.
`
`* Section 3 references the erosion control and other environmental measures required for the
`project,
`
`Section 1 - Major Project Elements
`
`Thus project, known as the Yelm Hydroelectric Project, Fish Screen and Spiflway No. 3, includes the
`following major work activities:
`
`* Atthe site of the existing Centralia Dam on the Nisqually River, an 800-cfs fish screen in the
`Centralia Canal will be constructed. This strecture includes a concrete canal section, flat plate
`wedge wire screen panels, screen cleaners, control gates, 38-inch HDPE bypass piping and a
`concrefe check structure, demolition of the existing fish screen mechanism and installation of a
`trash rake.
`
`* At the site of the existing powerhouse and Spillway No. 3, a new spillway will be constructed and
`shall include a new concrete spillway structure, spillway gates, large diameter welded sieel pipe
`spillway and concrete stilling basin. After completion of the new spillway, the existing spillway
`will be blocked and backfilled.
`
`Section 2 - Quality Control and Inspection Program
`
`During construction of the project, the City of Centralia will have one full time construction manager
`onsite to monitor construction. This person will be responsible for maintaining the overall quality
`control and inspection and testing program and project administration. His duties will include overall
`censtruction inspection, Reld tests, coordination of commercial testing labs, coordination of special
`inspechions by the engineer, preparing daily logs and submittal of these and other pertinent inspection
`reports to the Federal Energy Regulatory Commission (FERC) on a regular basis.
`
`The City’s consulting engineer, CH2M HILL will also provide special inspection as required on
`specific technical issues. These inspections will focus on geotechnical construction issues and other
`specialty inspections to ensure compliance with the contract documents and the City's” project
`requirements,
`
`Pertodic inspections will also be made by FERC staff during construction. The City's construction
`manager will inform the agencies of when this construction is occurring in order to coordinate the
`timing of these inspections.
`
`

`
`A project field office will be established on the project site. This office will be used by the
`construction manager for all field office work, including maintenance of quaiity control and contract
`administration records, meetings, and storage of field testing equipment.
`
`The construction manager's project involvement will begin with a thorough understanding of the
`contract documents. The construction manager will attend the preconstruchon meeting and will then
`monitor the progress of the work from notice-to-proceed through completion. The frequency of
`observation will be as needed to adequately measure contract compliance.
`
`The quality control testing required for this project is described in detail in the contract documents
`and should be referred to directly for specific information. A listing of the major tests and the
`associated inspection responsibility is listed below.

`Certification that the material conforms to the specification requirements along with copies of the
`test results from a commercial testing laboratory shall be submitted to the Engineer for approval
`al least 5 days before the material is required for use. No imported materials shall be delivered 1o
`the site until the proposed source and materials test have been tentatively accepted in writing by
`the engineer. Final acceptance will be based on test made on samples of material taken from the
`completed and compacted course. The completed course is defined as a course or layer that is
`ready for the next layer or the next phase of construction. All testing for final acceptance shall be
`performed by the Engineer,
`
`Gradation test shall be made on samples taken a1 the place of production prior to shipment.
`Samples of the finished product for gradation testing shall be taken from each 1,500 tons of
`prepared materials or more often as determined by the Engineer, if variation in gradation is
`otcurring, or if the material appears to depart from the Specifications. Test results shall be
`

`forwarded to the Engineer within 48 hours after sampling.
`
`If tests conducted indicated that the material does not meet specification requirements, material
`placement wall be terminated until correchive measures are taken. Matenial which does not
`conform to the specification requiremenits and is placed in the work shall be removed and
`replaced at the Contractor’s sole expense. Sampling and testing performed by the Contractor
`shall be done at the Contractor’s sole expense.
`
`Casi-in-Place Concrete and Grout
`
`All concrete and grout shall be batched in a prequalified manual, semi-automatic, or automatic
`plant. All concrete production facilities will be subject to verification inspections at the
`discretion of the Engineer. Verification inspections are a check for: current scale certifications:
`accuracy of water metering devices; accuracy of the batching process; and venification of coarse
`aggregate quality for Contractor-provided designs.
`
`Concrete cylinders for strength testing will be made, cured and tested in accordance with ASTM
`C31 and ASTM C39. Samples shall be taken, at least once a day, with intervals between testing
`not exceeding 150 cubic vards or 5000 square ft of surface area for walls and slabs. For sach
`sample four test cylinders shall be made. Testing will be conducted at 7 days, 14 days and 28
`days the fourth cylinder will be cast as a spare. The frequency of testing may changed at the
`discretion of the construction manager. Concrete samples for slump (ASTM C143-90a) and test
`cylinders {ASTM C31 and ASTM C39) shall be taken at the placement or discharged point for
`
`pumped concrete,
`Steel Pipe Welding & Fabricated Specials
`
`All pipe, linings, welds and related work shall be subject to inspection at the place of manufacture
`andfor the place the work is performed in accordance with the provisions of ANSFAWWA C200,
`C205, C206 AND C602. After the joint configuration is completed, each length of pipe of each
`diameter and pressure class shall be shop-tested and certified to a pressure of at least 80 percent
`of the yield strength of the pipe steel. Test pressure shall be mamtzined for a minimum of 5
`minutes. Production weld tests as required in ANSVAWWA C200, except weld tests shall be
`conducted on each 5,000 feet of production welds at a minimum. Additionaily, at least one set of
`tests per operator per work shift shall be performed.
`
`As 500n as practicable after the welding of each joint, all field-welded joints shall be 100 percent
`visually inspected by a certified welding inspector, Visual acceptance criteria shall be in full
`conformance with ANSIVAWS D1.1, Paragraph 10.17.5. All defects shall be chipped out,
`rewelded, and refested,
`
`Electrical Testing
`
`The project’s elecirical equipment and components shall be tested by a qualified electrical testing
`laboratory. The tests and inspections shall establish that 1.) All electrical equipment is
`operational within industry and manufacturer’s tolerances, 2.) Installation operates properly, 3.)
`Equipment is suitable for energization, 4.) Installation conforms to the requirements of the
`Contract Documents and NFRA 70, NFPA 70E, and ANSI C2.
`
`The inspection and testing shall be performed in accordance with NETA ATS, industry standards,
`and manufacturer’s recommendations.
`
`

`Section 3 - Organization
`
`This section outlines the construction project team organization and responsibilities of individual
`leam members. A project organization chart is shown in Figure 1.
`
`City of Centralla - The City"s staff members and their involvement in the project is as fallows:
`
`Mr. William C. Cummings, Light Dept. Director - Mr. Cummings will be the principal in charge of
`the project and will report directly to the City Council,
`
`Mr. Ron Whitman, Light Dept. Generation Manager - Mr. Whitman is responsible for all activities at
`the Yelm Hydroelectric Project and for direct communtcations with FERC Regional staff.
`
`Mr. Jan Stemkoski, City Engineer - Mr. Stemkoski will assist Mr, Cummings in engineenng matters
`and will ensure that qualified staff is available as needed for project inspections and will oversee Mr,
`Roe's work on the project. He will be onsite approximately two days a week to observe
`construction. He will also prepare monthly reports to Federal Energy Regulatory Commission
`(FERC).
`
`Mr. Curtis Roe, Construction Manager - Mr. Roe will be the construction manager for the City and as
`such will be the main pomnt of contact with the Contractor. He will be responsible for ensuring that
`the Contractor completes the project in accordance with the Contract Documents, He will perform
`construction inspection, field test, coordination of commercial testing laboratories, coordination of
`special inspection by the engincer, prepare daily inspection reports, and submit these and other
`pertinent inspection reports to FERC on a regular basis. He also has the authonty to stop work if
`required for any reason.
`
`CHIM HILL - As design engineer, CH2M HILL will provide Services During Construction (SDC)
`and have responsibility for office engineering and periodic site inspections to assist Mr. Roe in
`ensuring the quality of the constructed project. StafT assigned to the project will include;
`
`Robert Gation, Project Manager - Mr. Gatton has overall responsibility for providing technical and
`managerial support and staffing for the project.
`
`Shirfi Goto, SDC Manager - Mr. Goto will be the main point of contact with Mr, Roe. He will
`coordinate all of the CHZM HILL activities in the office and will visit the site at the biweekly
`meelings 1o stay current on construction progress as well as for required special inspections. He will
`observe structural steel installation and pipe welding on a periodical basis to verify conformance with
`the design.
`

`The following is the anticipated schedule for contractor sclection and construction of the project.
`This schedule will be replaced with the contractor’s schedule after its development and review,

`Section 5 = Erosion Control and Environmental Measures
`
`A separate document titled " Erasion and Sediment Control Plan™ has been prepared to specifically
`address the erosion and sedimentation controls and environmental measures required for this project.
`The intent of this document is to have the Contractor adopt and act on this plan. The Contractor shall
`submit a plan to the City based on the contents this document with medification for exact locations of
`the sedimentation pond as required to facilitate construction.
`
`This decument should be referenced for further information regarding this subject.
