`Evaluation Narrative
`
`I arrived at the facility at 13:55 on May 22, 2018 and after passing through security met with Nikki Riley,
`Senior Safety & Environmental Engineer. I explained that I was there to inspect Areas P and l as part of a
`RCRA Hazardous Waste Compliance Evaluation Inspection (CEI). After explaining the purpose of this
`inspection, Ms. Riley and I briefly discussed the processes at this facility. This facility manufactures
`medium caliber munitions, mostly 20, 25, and 30mm rounds. These rounds are available in a range of
`specialty loads (detailed on their website httpsfiwwwgd—otscomlmunitionslmedium-caliber-
`ammunition[) and can be slightly modified as required by the customer. These munitions are assembled
`in mechanized lines with a ”deluge” flash suppression system. This deluge system is camera activated
`and immediately deploys a large amount of water in an area if the camera detects any flash to stop
`explosive propagation. Therefore, cameras are only allowed in these areas if the flash has been
`
`thoroughly disabled, and even then, photographs in these areas are discouraged. These manufacturing
`lines are isolated from each other by use of blast walls and physical barriers. When these munitions are
`manufactured they are tested and if they do not meet the required specifications they are sent to an on-
`site Quality Analysis (QA) lab for a determination. The QA lab then determines if the rounds can be sold
`to a different customer, recycled as scrap for reuse at this facility, or is waste. Due to the wide variety of
`rounds that can be manufactured and the different customers it is difficult to predict which wastes will
`be generated. Another waste stream is pre-mixed propellant blends that are determined to no longer be
`of use. Other waste streams are generated via routine maintenance. After reviewing these processes, l
`. began with a review of the documents. This facility has notified as a Large Quantity Generator (LQG),
`‘
`and to verify this I began with the manifests. The manifests appeared to be in good order. General
`Dynamics uses a database with detailed information about the source of the waste streams, how much
`waste was generated monthly, the waste code, waste origination, waste disposal facility, profile
`number, and any additional notes. These records showed that this facility is 3 L06. This is consistent
`with the Hazardous Waste Reports submitted to the Agency.
`
`I then reviewed the preparedness and prevention policies and documents. A contingency plan was
`available, and Ms. Riley stated that this facility was being operated and maintained to minimize the risk
`of fire, explosion, or release of hazardous chemicals. An internal alarm system is present and detailed in
`
`the contingency plan, and intrinsically safe radios are issued to key personnel. Automatic fire
`suppression equipment has been installed and maintained, and adequate water volume and pressure is
`kept available. Arrangements with local emergency response were current. The plan appeared to be
`designed to protect human health and the environment, and described response actions for fires,
`explosions, and releases. Evacuation signals and‘routes were in this plan as well. The Emergency
`Coordinator information was up to date, and the most recent update had been provided to the Agency
`and local emergency response units. I was informed that this contingency plan was used when there was
`a flash in the mix house. No injuries occurred, and the area was evacuated and closed until the cause
`was determined, but no other actions were required.

`After reviewing the contingency plan and related documents I reviewed the training records. The job
`descriptions for each position that handles hazardous waste is maintained in the union contract and in
`the individual contracts of management personnel. These were available for review, as were the initial
`and annual training records. The training syllabi were also provided for review.
`
`The walk-through of P-area of this facility was conducted and was concluded at 17:00. Since this was
`well past the end of Ms. Riley’s shift and approaching the end of mine, I left the facility for the day. I
`returned at 8:35am on May 23, 2018 to conduct the inspection of I-area. This area is surrounded by a
`fence, and security checkpoints are at all active access points. The production lines couldn't be
`photographed due to safety reasons, but the hazardous waste accumulation areas were documented.

`The observed waste containers were labeled, and the non-satellite accumulations had start dates on
`
`them. All these accumulations start dates were within the 90-day window. A bucket of used oil dry was
`observed to be uncovered (photo 2), but this was corrected'during the inspection. There was a question
`about aisle space in a warehouse that is being re-organized, but that was also addressed during the
`inspection. No other potential violations were observed. This inspection ended at or round 10:15.
