`Please be advised that we have scheduled the Plaintiff, Capreese Moore, for an

`examination with Vito Loguidice, MD, on Friday, April 5, 2024, at 9:00 a.m. The Doctor’s
`office is located at Weyland Chiropractic, 20 Park Drive, Hornell, NY 14843. Kindly have Ms.
`Moore arrive 15 minutes early and bring with her any x-rays, lab reports, films, MRI’s, EMG’s,
`or any other diagnostic studies in her possession and her driver’s license. Please also have Ms.
`Moore bring a mask as she may be required to wear it while in the doctor’s office.
`
`Please be advised that if your client fails to cancel/re-schedule the above appointment
`
`within three business days of the scheduled date, there will be a missed appointment fee
`assessed in the amount of $495.00 for which we will seek reimbursement.
`
`Please contact the undersigned if there is an issue with this arrangement. Otherwise, we
`
`will expect your client’s attendance at this examination.
