`I, Rupert Lee of The British Library, 96 Euston Road, London NW1 2DB,
`United Kingdom, do solemnly and sincerely declare, that:

`I am an Information Expert at the British Library (the Library) and have
`worked for the Library for 26 years. I have access to and knowledge of
`the records and record keeping practices and procedures of the
`Library.
`I have reviewed these records and record keeping practices to
`prepare this declaration. Based on that review, the ordinary practices
`and procedures of the Library and my personal knowledge and belief, I
`make the following declaration.

`. The Reading Rooms of the Library are open to any bona fide enquirer
`and any publication in the Library may be consulted in the Reading
`Rooms on request. Any organisation or individual may purchase
`photocopies of extracts from publications.
`
`On the 8"‘ of July, 1993 the Library received a copy of Power Plant
`Health Monitoring — The Human Factor, by M. J. Ward, a paper
`originally presented at the 10 Annual Symposium of the Royal
`Aeronautical Society, New Zealand Division, on 13th-14"‘ February
`1992. It was to the best of my knowledge received intact and was
`assigned the Library shelfmark 8019322000.
`It was then catalogued
`on 26"‘ of July, 1993 and would have been available for public use to
`the best of my knowledge and belief from that date. There is now
`produced to me and marked “A" a true and correct copy of the cover
`page showing the Library pressmark and receipt date stamp and the
`cataloguing date stamp‘.
`
`. The Library pressmark and. receipt date stamp and the cataloguing
`date stamp were made at or near the time of the matters set forth
`therein, by—or from information transmitted by—a person with
`knowledge. These records were kept in the course of a regularly
`conducted activity of the Library. These records were made as a
`regular practice in the course of a regularly conducted activity.
