`issued to me as the sole and exclusive inventor of such inventions:

`0 US. Patent 6,712,832 issued to me on March 30, 2004 for “LOW-PRESSURE MEDICAL
`BALLOONS AND METHOD OF MAKING SAME”

`- US. Patent No. 7,682,306 issued to me on March 23, 2010 for “THERAPEUTIC
`INTERVENTION SYSTEMS EMPLOYING IMPLANTABLE BALLOON DEVICES”
`I US. Patent 7,883,491 issued to me on February 8, 2011 for “EXTRUSION LAMINATE
`POLYMERIC FILM ARTICLE AND GASTRIC OCCLUSIVE DEVICE COMPRISING
`SAME”
`
`The above-referenced inventions and US. Patents are hereinafter referred to as the “Patent Rights.”
`
`I hereby assign to Polyzen, Inc., 1041 Classic Road, Apex, NC 27539, all my ownership, rights, title,
`and interest in and to the Patent Rights, including the right to sue for past infringement thereof.
`
`I am making this assignment based on my business relationship with Polyzen, Inc. and for other good
`and valuable consideration, including the undertaking by Polyzen, Inc. to administer and enforce the
`Patent Rights.
`
`I respectfully request the Commissioner for Patents of the United States to recognize Polyzen, Inc. as the
`owner of the Patent Rights pursuant to this assignment.
`
`I agree to execute any other legal documents that may be required to effectuate this assignment.
`
`I agree that I will promptly communicate to Polyzen, Inc. all facts which come to my attention affecting
`the Patent Rights.
`
`I agree that I will testify in any interference or litigation involving the Patent Rights.
`
`I declare that I have not done anything in conflict with this assignment.
