`MOTION TO ADJUDICATE GUILT
`
`COMES NOW, THE STATE OF TEXAS, by and through the undersigned Assistant District Attorney and shows
`
`the Court heretofore on 8th day of July, 2020, the Defendant herein entered a plea of Guilty for the Class A
`Misdemeanor offense of THEFT PROP >=$750<$2,500, and the Court after hearing the evidence introduced
`thereon and finding that it substantiates the Defendant’s guilt, deferred entering an adjudication of guilt and
`placed the Defendant under the terms and conditions of Community Supervision for a period of 1 Year in
`accordance with Section 5 of Article 42.12 of the Texas Code of Criminal Procedure.
`
`Further, the State would show that the Court ordered the Defendant herein to abide by certain conditions of
`Community Supervision during the term of Community Supervision and among the conditions of Community
`Supervision ordered by the Court were the following conditions of Community Supervision.
`
`The State would further show the said Defendant did then and there violate terms and conditions of
`
`Community Supervision by: Failing to report to the Community Supervision Officer, to-wit; the Defendant
`failed to report as directed on July 21, 2020 and has not reported to her Community Supervision Officer
`since.
`
`The State would further show the said Defendant did then and there violate terms and conditions of
`
`Community Supervision by: Failing to present verification of employment, to-wit; the Defendant failed to
`present written verification of employment for the month/months of July 2020.
`

`The State would further show the said Defendant did then and there violate terms and conditions of
`
`supervision by: Failing to submit to an evaluation of the education skill level as directed by the CSCD on
`the following date(s): August 8, 2020.
`

`The State would further show the said Defendant did then and there violate terms and conditions of
`
`Community Supervision by: Failing to pay supervision fee as directed by the Court and is $80.00 in
`arrears as of 09/01/20. The Defendant has never made a payment.
`

`The State would further show the said Defendant did then and there violate terms and conditions of
`
`Community Supervision by: Failing to pay Client Identification Card fee as directed by the Court and is
`$0.00 in arrears as of 09/01/20.

`The State would further show the said Defendant did then and there violate terms and conditions of
`
`Community Supervision by: Failing to pay Court Costs as directed by the Court and is $25.00 in arrears
`as of 09/01/20.

`The State would further show the said Defendant did then and there violate terms and conditions of

`Community Supervision by: Failing to pay Crime Stoppers of Houston fee as directed by the Court and is
`$50.00 in arrears as of 09/01/20.

`19. The State would further show the said Defendant did then and there violate terms and conditions of

`Community Supervision by: Failing to pay a drug testing fee as directed by the Court and is $20.00 in
`arrears as of 09/01/20.

`21. The State would further show the said Defendant did then and there violate terms and conditions of
`
`Community Supervision by: Failing to pay Assessment fee as directed by the Court and is $100.00 in
`arrears as of 09/01/20.

`WHEREFORE, THE STATE PRAYS that Alias Capias issue and upon arrest that a hearing be given the
`Defendant and that on the final hearing an adjudication of guilt be entered.
