`for the period during Which Petitioner’s license has been suspended. Petitioner may also:
`Operate a motor vehicle in the performance of an occupation or trade or for transportation
`to and from the place at Which Petitioner practices said occupation 0r trade.

`IT IS FURTHER ORDERED that Petitioner shall attend alcohol group counseling or
`
`alcohol individual counseling once weekly for _____ weeks, with the first week beginning the first
`Monday after the date of this Order, at the location indicated as follows (Petitioner shall initial one

`This Order shall take effect immediately, and Petitioner may use a copy of this Court Order
`as an occupational Texas driver’s license for 45 days after the date this Order is signed.
`
`IT IS FURTHER ORDERED that Petitioner maintain a valid policy of automobile liability
`insurance in accordance with the provisions of applicable Texas law for the period during which
`Petitioner’s license has been suspended, and that Petitioner shall carry a certified copy of this Order
`at all times while operating a motor vehicle. Failure of Petitioner to comply with this Order will
`result in this license being revoked without further notice.

`The applicant is ordered to only operate vehicles with an interlock device supplied
`
`by Smart Start.
