`Last week, we sharedinformation with you about the masking requirement for Associates and providers.with
`ppiayes exemptions that will become effective November1.
`Regular COVID testing is also required of all Associates and providers with approved vaccine exertions as part of
`our enhanced safety measures to accommodate these exemptions. We havefinalized the details relating to our:
`COVID-19 testing requirement. Please review these details carefully so you can be preparedto take action, beginning
`the week of Nov.1.

`Associates andproviders with approved exemptionswill be required to undergo COVID testing from the
`approved COVID test providerat least twice weekly. There must be more than 48 hours between the twoweekly...
`Testing will beavailable through Poplar Healthcare attheir location at 3495 Hacks Cross Road.
`You must test at the Poplar Healthcare drive thru. MLHwill not accept COVID-19 tests from other testing
`Testing must occur on personal time, not during working hours and youwill not be paid for time spentgetting
`Poplar Healthcare does not take appointments.Their testing drive thru will be openfor MiH onn Mondays and _
`Thursdays from 6am'to’6pm-and Wednesday, Friday and Saturday from 8am to 6pm.
